Community Thrive: Health, Independence, and Community Support
At EPTSAL, we know that health and independence are the foundation of thriving communities. That’s why we created Community Thrive — an umbrella program that combines two vital initiatives: Senior Thrive and Breathe Again.
Together, these programs address the most pressing needs of our most vulnerable neighbors — ensuring seniors, veterans, individuals with disabilities, and respiratory patients receive the support, education, and resources they need to live fuller, healthier lives.
🌿 Senior Thrive – Mobility & Independence for Seniors
Senior Thrive is designed to remove barriers that prevent independence, safety, and dignity. We serve underinsured and uninsured seniors, veterans, and people with disabilities by providing:
🚗 Transportation Support – rides to doctor visits, pharmacies, and grocery stores.
♿ Mobility Equipment Access – wheelchairs, walkers, and canes for those in need.
😁 One-Time Dental Support – including dentures and urgent care to restore smiles and health.
Our Goal: Ensure every senior can live independently, access care, and participate fully in community life.
💨 Breathe Again – Respiratory Health & Caregiver Education
Breathe Again empowers patients with chronic respiratory conditions to take control of their health, stay out of the hospital, and live with greater confidence.
Through partnerships with licensed respiratory therapists, pulmonologists, and accredited educational resources, we provide a four-week program that includes:
Proper inhaler use and medication education
CPAP cleaning and tracheostomy care training
Pneumonia prevention strategies
Caregiver education and hands-on support
Ongoing patient monitoring until independence is achieved
Our Goal: Reduce hospital readmissions, strengthen caregiver support, and improve quality of life for respiratory patients.
